Its happened to me a few times now. I've managed to cram 22 items into my backpack! I think it may have something to do with trading with other players, but not 100% sure...
Its always happened to me where the red zombies are. I have a screenshot I can post if needed.

This is the trade screen. In backpack it only shows 21, but if I use or lose one, I still have 21.

yeah i've see a similar bug: u can enter 1item more when 21 full...strange
