I don't ever recall getting a drop from an enhanced monster. Is this the norm or me being incredibly unlucky?

I may be mistaken, but I believe the superior monsters (enhanced as you put it) only have improved stats, but since they do not share the same monster ID, they are not on the drop list as the original monster is. Therefore, I believe you are correct to some extent. Each particular superior monster would need to be added to the drop list as a separate entry, and I do believe some have been added, I don't think they all have.

I've died in several time but i didn't drop any item

There are no item drops from you if you are killed by a monster.

Yes. You need to kill the monster rather than the other way round

Lol. I meant that a player will not lose anything but experience if killed in PvM.
