i killed 15 red spiders north of beginning map. lady doesnt talk to me.

Have you spoken with her before and are you level 2? I guess the answer to both is yes, but they are quest prerequisites.

I killed 5 more just to be safe, the question mark above her head has disappeared

You have a quest to kill spiders and bring 15 spider exoskeletons to the lady? You need to have 15 of those in your backpack before you speak with her for the reward.
