I have an idea, also comment if it is possible to implement it!


An island with a lot of acid and polluted air appears in the game!
Therefore, to be there, players need to use equipment - such as a sphere in which oxygen will be placed and a magic mask that interacts with the sphere.
There are 3 levels of spheres - 1 for 15 minutes, 2 for 25 minutes, 3 for 45 minutes!
To get to the island you can get level 1 gear from the shop!
Higher level gear can be crafted on the island using monster drop items!
An oxygen potion will also be created to replenish the oxygen supply in the sphere!

If the oxygen supply runs out, the player will get poisoned and die!

I'd just avoid the island:)

Sounds similar to Omega Vanitas.

I'm not averse to damage over time scenarios, but this would really need to be thought through carefully.
